The only one you didn't get is a little obscure:

Umm, would it help to say that it's a young adult novel? The author has written quite a number of stories and one of them (not this one!) got turned into an animated film directed by Hayao Miyazaki.

Yes, it's one of hers. I don't think its widely-read, but it's Polly/Thomas Lynn from Fire and Hemlock (http://en.wikipedia.org/wiki/Fire_and_Hemlock). It's really well-written and a cool read.
